Participation in PHP occurs through membership in Purchase Physicians’ Organization (PPO). PPO was established in 1994 and converted to LLC status in 1997. To become a member contact (270) 415-7227 or (270) 415-7230 to receive a credentialing application.

• Participating provider - PHYSICIANS ONLY. Initial credentialing fee of $150 then an annual assessment of $150 per year thereafter. No recredentialing fee.
• Equity member - Participating providers may also become equity members which entitles the physician to vote on matters presented to the participating membership. There is a one time fee of $200 for PCP or $400 for specialists.
• Preferred participating provider - NON PHYSICIANS. Initial credentialing fee of $150 then an annual assessment of $150 per year thereafter. No recredentialing fee.
• All providers are recredentialed every three(3) years.
